Role Summary

East Cleveland PCN Ltd is seeking a dedicated Pharmacy Technician to join our multi-disciplinary team. The successful candidate will play an essential role in enhancing clinically and cost-effective prescribing across our network population. This role is crucial in improving patient outcomes and alleviating service and financial pressures within the PCN.

Key Responsibilities
  • Clinical Medication Reviews: Manage care for patients with chronic diseases, conducting medication reviews to aid in the proactive management of complex polypharmacy cases. This includes elderly individuals in care homes, patients with multiple co-morbidities, and those with learning disabilities and autism.
  • Medicines Optimisation: Support person-centred medicines optimisation, standardising care approaches across the PCN, and fostering relationships with specialist and community pharmacists.
  • Prescription Support: Provide day-to-day assistance to practices regarding prescription and medication queries. Manage the repeat prescription system, handle acute prescription requests, and perform medicines reconciliation during care transitions.
  • Education and Progression: Engage in the CPPE Primary Care Pharmacy Education Pathway, enhancing knowledge and skills in patient-facing, person-centred, and integrated care.
Qualifications and Skills
