Job summary
Towork as part of the general practice team to optimise medicines use, supportsafe prescribing, and improve patient care. The pharmacy technician will play avital role in medicines reconciliation, repeat prescription management, andpatient education to ensure safe, effective, and efficient use of medicines.
Key Duties & Responsibilities
Carryout medicines reconciliation following hospital discharge, outpatient clinics,or care home transfers.
Supportrepeat prescription processing, ensuring appropriate reviews, accuracy, andcost-effectiveness.
Supportpharmacist with project work
Reviewpatients compliance with treatment, encouraging safe and effective medicinesuse.
Provideinformation and advice to patients, carers, and colleagues on medicines andminor ailments.
Carryout housekeeping whilst in patient records e.g. dose optimisation and medicines synchronisation.
Assistwith chronic disease management by supporting long-term condition reviews underthe supervision of a pharmacist/GP.
Workclosely with GPs, practice nurses, and pharmacists to ensure safe prescribingand reduce medicines-related risks.
Supportmedication safety initiatives, audits, and quality improvement projects.
Ensureprescribing follows national guidance (e.g., NICE, local formularies) andpractice policies.
Contributeto repeat prescribing reviews, reducing waste and promoting cost-effective useof NHS resources.
Liaisewith community pharmacies, hospitals, and care homes to ensure continuity ofcare.
Participatein training, supervision, and continuous professional development.
Participatein practice meetings, patient participation groups, and other meetings to improve engagement of the role of pharmacytechnician
Ensure that all major MHRA drug alerts, whereappropriate and within level of competence, are actedupon within practices over an appropriate time frame and to advise on anymedication changes that occur as a result.
Contributeto the achievement and maintenance of good to outstanding CQC registration status.
