Bay Medical Group are looking to recruit a Pharmacist on a part time basis, working 24.5 hours per week.This is a permanent position and work pattern will be Monday 9 till 2:30, Wednesday 8 till 6 and Friday 8 till 6 and will include a 30 minute unpaid break.

Main site of work will be Heysham Primary Care Centre however travel across our 5 sites is required.

Main duties of the job

They will play a crucialrole in supporting the repeat prescription system, handling acute prescriptionrequests, conducting medicines reconciliation during transfers of care, andimplementing systems for safer prescribing. Additionally, they will offerexpert clinical guidance on medicines, addressing the public and social careneeds of patients.

The post holder willtake a leading role in medicines optimisation and quality improvement,overseeing aspects of the quality and outcomes framework as well as enhancedservices.

Ensuring seamlessintegration between community and hospital pharmacies, the post holder willcontribute to the efficient use of skill mix, enhancing patient outcomes,improving healthcare access, and helping to manage workload. This role is vitalin driving improvements in both the quality of care and operationalefficiencies, requiring a high level of motivation and a passion for deliveringexceptional service within general practice.

Primaryduties and areas of responsibility

Clinics/Clinicalassessment (Tasks/AccuRx)

Conduct both face-to-face and telephoneconsultations with patients diagnosed with one or more medical conditions,where medicinal intervention is necessary (e.g., COPD, asthma).

For post holders who are independentprescribers, prescribe safe, effective, and appropriate medications inaccordance with current clinical guidelines and relevant legislation.

Clinical medicationreview

Conduct clinical medication reviews withpatients, providing recommendations to senior clinical pharmacists, nurses,and/or GPs regarding prescribing and monitoring decisions.

Assess the ongoing necessity of eachmedication, review monitoring requirements, and use each consultation as anopportunity to support patients in managing their medication effectively,ensuring optimal use of their medicines through medicines optimisation andstructured medication reviews.

Care home medication reviews

Conductclinical medication reviews for patients, collaborating with care home staffand community pharmacists to enhance the safety of medication ordering andadministration processes.

Domiciliary clinical medication review

Undertake clinical medication reviews withpatients, providing recommendations to senior clinical pharmacists, nurses, andGPs regarding prescribing and monitoring.

Attend multidisciplinary case conferences andrefer patients as needed.

Management of common/minor/self-limiting ailments

Manage a caseload of patients with common,minor, or self-limiting conditions, working within the scope of practice andlimits of competence.

Signpost patients to community pharmacies andrefer to GPs or other healthcare professionals when necessary.

Repeat prescribing

Oversee thepractice's repeat prescribing policy and reauthorization process.

Ensure thatpatients undergo necessary monitoring tests when required.

Identifypatient cohorts at high risk of harm from medicines through pre-preparedpractice computer searches, considering both patient- and medication-relatedrisks.

Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (e.g. advice on treatment pathways and patientinformation leaflets).

Information management

Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making.

Conductclinical audits of prescribing practices, providing feedback and implementingimprovements in collaboration with the practice team.

Implementchanges in response to MHRA alerts, product withdrawals, and other local or nationalguidance to enhance medicines safety.

Implementation of local and national guidelines andformulary recommendations

Monitorpractice prescribing against the local health economys RAG list and makerecommendations to GPs for medicines that should be prescribed by hospitalclinicians (red drugs) or subject to shared care (amber drugs).

Assist thepractices in maintaining the practice formulary that is hosted on thepractices computer system.

Audit the Practicescompliance against NICE technology assessment guidance.

Providenewsletters or bulletins on important prescribing messages.

Education and training

Provideeducation and training to the primary healthcare team on therapeutics andmedicines optimisation.

Care Quality Commission

Collaboratewith the general practice team to ensure compliance with CQC standards relatedto medicines management.

Public health

Supportpublic health campaigns and provide expert knowledge on available public healthprogrammes.

Collaborative working relationships

Knowledge, skills and experience required

Hold anundergraduate degree in pharmacy and be registered with the GeneralPharmaceutical Council.

Person Specification

Experience

• Experience in working with computerised health care systems.
• At least 1 years post qualification experience in General Practice, hospital or community pharmacy.
• Previous experience of working in triage or unscheduled care service.

Qualifications

• Registrant of General Pharmaceutical Council (GPhC) and commitment to CPD.
• Member of Professional Body e.g. Royal Pharmaceutical Society.
• Hold post graduate clinical Diploma or equivalent.
• Independent prescriber.
• Completion of either the Primary care pharmacy education pathway, or MSc Advanced Practice Pathway.
