• Experience working in paediatric nephrology.

Responsibilities

• Prepare, assess, train and follow‑up children and their families.
• Manage caseload of CKD patients.
• Prepare children and families for transplant listing.
• Organise regular follow‑up via telephone, face‑to‑face clinics, home visits and shared care clinics.
• Undertake home assessments.
• Train patients and carers in related therapies.
• Liaise with primary care providers, schools and local authorities.
• Coordinate supplies and medication.
• Respond to family advice calls, troubleshoot issues, and escalate concerns.
• Work regular shifts in the children's haemodialysis unit.
