Job description

We are seeking an enthusiastic andmotivated Pharmacy Technician to join our dynamic team. This is an excellentopportunity to contribute to the delivery of high-quality medicinesoptimisation, support chronic disease management, and help improve patient careacross the PCN.

Main duties of the job

Supportpharmacists in the delivery of clinical and administrative pharmacy services

Assistwith medicines optimisation and long term condition management

Conductstructured medication reviews

Optimisemedication based on hospital discharge letters

Answermedication and prescription queries from patients and other healthcareprofessionals

Supportthe prescription team in day to day tasks

Supportimplementation of national policies and guidelines

Contributeto reducing medication wastage

Participatein face to face clinics in competent clinical fields (full training provided)

Receiveone to one clinical supervision and support from clinical pharmacists

Takepart in ongoing training and career progression opportunities

Helpreduce GP workload through effective pharmacy interventions

Ensurecompliance with relevant standards, including CQC, NICE, MHRA, QOF, and PCN DES

Assistwith patient education and adherence support for long-term conditions

Contributeto audits, quality improvement projects, and service evaluations

Assistwith clinical coding and data collection

Participatein multi-disciplinary team meetings and collaborative care planning

Supportmedicines reconciliation in patients transitioning between care settings

About us

Victoria Primary Care Network (VPCN) provides care for approximately 50,000 patients across Victoria Medical Practice, Downlands Medical Centre and Vita Healthcare Practices.

The PCN pharmacy team comprises three pharmacists and four pharmacy technicians.
