Job Summary

We have an exciting opportunity in our Primary Care Network for a clinical pharmacist to join our team. The role will support in developing medicines optimisation services within our Network. Morley has a well-established PCN made up of 6 GP practices covering a population of 63,000. The role may be working across any site within our PCN.

About Us

We are a 6 practice PCN with approximately 63,000 patients. We are rapidly developing our multi‑disciplinary workforce, embedding our roles, developing our team that makes a real difference to our patients and our practices.

Job Responsibilities
  • Undertake clinical medication reviews with patients and produce recommendations for nurses and/or GP on prescribing and monitoring.
  • Review the on‑going need for each medicine, monitoring requirements and provide support for patients with their medicines.
  • Identify high‑risk patient cohorts for medicine harm through computer searches.
  • Conduct clinical audits of prescribing in areas directed by the GPs, feedback results and implement changes.
  • Manage medicines at the transfer of care, reconciling medicines following discharge from hospitals and ensuring continuity for high‑risk groups.
  • Provide medicines support for patients with questions, queries and concerns.
  • Support and work alongside the Lead PCN Clinical Pharmacist, Pharmacy Technician, Prescription Teams and Dispensary Teams.
  • Promote prescribing quality and safety, support repeat prescribing policy and manage reauthorisation processes.
Person Specification
  • Minimum of 2 years post‑qualifying experience in pharmacy practice.
  • Knowledge of the wider determinants of health and primary care prescribing.
  • Excellent written and verbal communication skills and the ability to influence and motivate.
  • Experience of partnership/collaborative working across a variety of organisations.
  • Understanding of IT systems, including use of word processing, emails and the internet.
  • Confidentiality, safeguarding, lone working, information governance and health and safety experience.
Qualifications
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Mandatory registration with the General Pharmaceutical Council.
  • Holds an independent prescribing qualification.
Desirable Qualifications
  • Membership of PCPA.
  • Membership of the Royal Pharmaceutical Society.
  • Clinical Diploma or equivalent.
Experience
  • Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio.
  • Experience and awareness of common acute and long‑term conditions in general practice.
  • Experience of partnership/collaborative working and building relationships across a variety of organisations.
Desirable Experience
  • Experience working in primary care.
  • Experience working in a GP practice.
Knowledge and Skills
  • Minimum of 2 years post‑qualifying experience.
  • Understanding of social, economic and environmental determinants of health.
  • Appreciation of primary care prescribing concepts and rational prescribing strategies.
  • Knowledge of IT systems and ability to create simple plans and reports.
  • Ability to plan, manage, monitor and review general medicine optimisation issues for long‑term conditions.
  • Ability to gain acceptance for recommendations and influence motivation.
Desirable Knowledge and Skills
  • In‑depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare.
