A community healthcare organization in England is seeking a Patient Flow Coordinator to efficiently manage patient discharges in our hospitals. This role involves effective communication with patients, families, and a multidisciplinary team to ensure timely patient movement. Responsibilities include coordinating discharge processes, maintaining documentation, and participating in daily meetings. The Patient Flow Coordinator plays a pivotal role in ensuring the safe, timely, and efficient movement of patients through our community hospitals.
This is an exciting new role which will require you to communicate effectively with patients, families and our multidisciplinary team in order to help patients get home as soon as they are ready. You will work with the teams on our wards at Abingdon Community Hospital. Your work will directly contribute to reducing length of stay, preventing readmissions, and improving patient outcomes.
