A national health service in Swanscombe is looking for a Paramedic Practitioner to assess and manage patients with acute presentations. The role requires working autonomously within a multi-disciplinary team, providing treatment, referrals, and support to patients in their homes. Ideal candidates will have a degree in paramedicine and HCPC registration, along with excellent communication and organizational skills. We are looking for a Paramedic Practitioner to join our team at Swanscombe Health Centre and branch practices, towork independently within theirscope of practice in the community, using their enhanced clinical assessmentand treatment skills to assess and manage patients presenting with acutepresentations which include minor illness or injury, abdominal pains, chestpains and headaches.
The post-holder will bean integral part of the general practice team.
Based at Swanscombe Health Centre, the role of the Paramedic Practitioner will be to work autonomously but with support of our large team of clinicians, while visiting our patients within their own homes to offer assessment, diagnosis, treatment and continuing support. Some late afternoon/evening visits may be required within the working day.
You should have experience of working in a similar role, with relevant qualifications, HCPC registration, and have excellent organisational/communications skills.
We have a team of over 100 staff throughout 8 sites, including GPs, Paramedic Practitioner, Clinical Pharmacists, Physicians Associates, a Physiotherapist, Practice Nurses, Healthcare Assistants, Social Prescribers and the administrative support workers. Even with a large team, we work closely together and clinical staff are supported by a weekly clinical meeting.

The following are the core responsibilitiesof the Paramedic delivering health services.
a.Work as part of a multi-disciplinary team (MDT) withinthe organisation
b.Assess and triage patients, including same day triage,and as appropriate provide definitive treatment (including prescribing medicationsfollowing policy, patient group directives, NICE (national) and localclinical guidelines and local care pathways) or make necessary referrals toother members of the primary care team
c.Advise patients on general healthcare and promoteself-management where appropriate, including signposting patients to thesocial prescribing service and, where appropriate, other community or voluntaryservices
d.Be able to:
Perform specialist health checks and reviews within theirscope of practice and in line with local and national guidance
Perform and interpret ECGs
Perform investigatory procedures as required andundertake the collection of pathological specimens including intravenousblood samples, swabs and other samples within their scope of practice andwithin line of local and national guidance
e.Support the delivery of anticipatory care plans andlead certain services (e.g., monitoring blood pressure and diabetes risk ofelderly patients)
f.Communicate at all levels across organisations ensuringthat an effective, person-centred service is delivered
g.Communicate proactively and effectively with allcolleagues across the MDT, attending and contributing to meetings as required
h.Maintain accurate and contemporaneous health recordsappropriate to the consultation, ensuring accurate completion of all necessarydocumentation associated with patient healthcare and registration with theorganisation
i.Communicate effectively with patients and, whenappropriate, family members and their carers, where applicable, complex andsensitive information regarding their physical health needs, results,findings and treatment choices
j.Receive referrals and directed patients from triageservices and other clinicians
k.Maintain accurate clinical records in conjunction withextant legislation
l.Develop yourself and the role through participation inclinical supervision, training and service redesign activities
m.Actively signpost patients to the correct healthcareprofessional
n.Review the latest guidance, ensuring the organisationconforms to NICE, CQC etc.
o.Be aware of duties and responsibilities regarding currentlegislation and adhere to practice policies and procedures on safeguarding adultsand safeguarding children
p.Support in the delivery of enhanced services and otherservice requirements on behalf of the organisation
q.Participate in the management of patient complaints whenrequested to do so, and participate in the identification of any necessarylearning brought about through clinical incidents and near-miss events
r.Undertake all mandatory training and induction programmes
s.Contribute to and embrace the spectrum of clinicalgovernance
t.Attend a formal appraisal with your manager at leastevery 12 months. Once a performance/training objective has been set, progresswill be reviewed on a regular basis so that new objectives can be agreed
u.Contribute to public health campaigns (e.g., flu clinics)through advice or direct care
v.Maintain a clean, tidy, effective working area at alltime
Theremay be, on occasion, a requirement to carry out other tasks, this will bedependent upon factors such as workload and staffing levels.
