A leading healthcare provider in the UK is looking for a Palliative Care Clinical Nurse Specialist (CNS) Band 7. This role involves providing expert palliative care nursing advice, managing a caseload, and ensuring high-quality individualized care in a multidisciplinary team. An exciting opportunity has arisen for Palliative care CNS Band 7 to join our dynamic team. If you are passionate about Palliative care, but are not sure if the specialty is the right one for you, then this is the chance to work with the team and understand what it takes to be a Palliative care CNS on a fixed term contract.
As a specialist palliative care CNS you will provide expert palliative care nursing advice, to support and facilitate high quality, individualised care to patients and relatives/carers in collaboration with the multi professional team, in line with the local and national targets.
The successful candidate will be trained to manage own caseload of patients with appropriate input from Senior team, working as part of the multi-disciplinary Specialist Palliative Care Team. Maintain effective communication with patients, families/carers and healthcare professionals to ensure seamless service delivery.
As part of the specialist palliative care team, you will be supported to undertake comprehensive palliative care assessments identifying/diagnosing patients physical, social, psychological, cultural and spiritual needs; to facilitate the provision of quality nursing care and to contribute to decisions regarding clinical care and priorities. Attend or initiate case conferences as appropriate.
