Job Overview

Are you a passionate Nurse or Occupational Therapist looking to make a real difference? There's never been a better time to join us here at Derbyshire Healthcare NHS Foundation Trust! We're looking for someone with experience working with older adults in the community who are living with functional mental health conditions. This brand‑new role, based within the Older Adults CMHT, offers an exciting opportunity to shape meaningful support for individuals in our local area.

You will play a key part in introducing people into the Living Well Team, helping to identify their needs and designing person‑centred, compassionate support that empowers them to live their best life. Collaboration is at the heart of this role – you will work alongside colleagues across the community to ensure that the right support is delivered at the right time by the right person.

The role is supported by the Older Adults CMHT and you will also work closely with the Living Well Team, a multi‑agency group that places individuals right at the centre of their care.

Responsibilities
  • Introduce people into the Living Well Team and help identify their needs.
  • Design and deliver person‑centred, compassionate support for older adults with functional mental health conditions.
  • Collaborate closely with community colleagues to deliver timely and appropriate support.
  • Travel across the geographical area to meet clients and multidisciplinary teams.
  • Support the expansion of the Living Well Team by identifying individuals who may not have had access to mental health support before.
