What you will do:

  • Conduct assessments for adults with disabilities, ensuring interventions maximize independence and are cost-effective.
  • Manage your own caseload, collaborating closely with the People Directorate.
  • Work as an autonomous practitioner, accountable for your work and supervision.
  • Plan and provide interventions around manual handling, seating, and small aids in compliance with government policies.
  • Document all activities aligned with legislation and local procedures.
  • Engage in continuous professional development and attend all mandatory and specialist training sessions.
  • Participate actively in team meetings and contribute to service area staff training.
  • Involve yourself in safeguarding assessments and planning as required.

To excel in this role, you need to be HCPC registered and a car driver.
