Job Overview

Permanent part‑time Nursing Support Worker position on the Community Nursing Team, supporting the Community Diabetic Clinical Nurse Specialist in Carmarthenshire. The role operates as part of a multidisciplinary team, delivering high‑quality care in patients’ homes across the region.

Responsibilities
  • Provide high quality care to patients in their own homes as part of a nursing team, guided by the Community Diabetic Clinical Nurse Specialist.
  • Ensure high standards of individualised care are delivered to patients and their families and carers within the home environment, following care plans and treatment interventions delegated by the registered nurse.
  • Work competently without direct supervision, applying nursing knowledge and skills.
  • Travel between sites in a timely manner and work a 7‑day shift pattern, including weekends and bank holidays if required.
  • Support the health board’s bilingualism policy, communicating in Welsh where appropriate and helping patients and colleagues where necessary.
  • Actively participate in professional development and support the improvement of community nursing practice.
Qualifications and Requirements
  • Experienced and committed Nursing Support Worker with a professional background in community nursing.
  • Ability to work as part of a multidisciplinary team supporting a Clinical Nurse Specialist.
  • Capacity to manage independent duties and follow delegated care plans.
  • Willingness to travel between sites and work a flexible, 7‑day schedule.
  • Desirable: proficiency in Welsh. If Welsh language requirements are not yet met, the health board offers learning options.
