Overview

Derbyshire Community Health Services NHS Foundation Trust (DCHS) is one of the largest specialist community healthcare providers in England, caring for nearly 1.8 million patients each year through 11 community hospitals, 4 minor injury units, 29 health centres and a learning disability centre.

Responsibilities
  • Provide high quality nursing care to patients in the local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
  • Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way while maintaining the patient’s privacy and dignity.
  • Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.
  • Implement/refer to the relevant services as appropriate.
  • Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.
