Overview

Betsi Cadwaladr University Health Board in North Wales is a dynamic and patient-focused organisation. The Health Board is responsible for the direct management of several GP Practices and is developing these practices to be gold standard exemplars, creating services that meet the needs of the local population, are easier to access and navigate, and provide a rewarding and satisfying place to work.

Responsibilities
  • Development and delivery of high quality patient care within primary care practices.
  • Develop opportunities for the practice that extend the scope of traditional nursing.
  • Training and support of nurses, patients and the practice MDT to deliver high quality, evidence-based service provision.
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Demonstrate a contemporary approach to Primary Care that is patient-centred, prudent and team-enabled in day-to-day practice.
  • This role will require flexibility to meet the demands of the practice and an individual work plan will be agreed in advance with the successful applicant.
Qualifications
  • Experience as a Nurse Practitioner in Primary Care with evidence of Chronic Disease Management.
  • Registered first level Nurse with evidence of study at MSc level.
  • The ability to speak Welsh is desirable; English and/or Welsh speakers are equally welcome to apply.
