Overview
The emphasis of this service is to improve the quality of life for children and families with life‑limiting, palliative or complex health care needs, offering them greater choice around their chosen place of care.
Responsibilities
- Work in accordance with the NMC Code of Professional Practice and Scope of Professional Practice, being professionally and legally accountable for all activities.
- Work within the community (e.g., within the client’s own home) as a lone practitioner, managing situations of risk with telephone support and seeking advice from more experienced colleagues when faced with matters outside your sphere of responsibility, knowledge or skills.
- Provide a calm and supportive approach to the management and delivery of sensitive and complex information, offering both practical and emotional support, utilising flexibility and empathy in difficult situations.
- Use effective communication techniques to liaise with the multidisciplinary team, ensuring that children and families referred to Children’s Rapid Response receive the best possible care.
- Undertake risk assessments prior to service delivery within the home setting to ensure staff safety.
- Work in partnership with children, families and carers to provide optimum individual outcomes, acknowledging personal preferences, spiritual and religious beliefs.
- Formulate individualized management and treatment plans using sound knowledge and evidence‑based practice, responding to clinical reasoning and including expected signs of deterioration.
- Provide timely, effective symptom management based on comprehensive history taking, utilising independent non‑medical prescribing, GP or hospital team support, and completing symptom flow charts to monitor a child’s changing condition.
- Provide and promote strategies and detailed plans of care to parents and carers, empowering care that will prevent inappropriate admissions and reduce length of hospital stay.
- Ensure assessment, planning and delivery of holistic care is of the highest standard, utilising an agreed nursing model.
- Liaise and work in collaboration with acute services, primary health care teams and other agencies to facilitate early discharge and prevent admissions.
- Advise the Team Leader, Clinical Lead or Service Manager on any circumstances that raise concerns about patient care or any concerns raised by partner agencies.
- Maintain clear, concise and contemporaneous records, adhering to BCHC record‑keeping policies.
- Provide a flexible service to accommodate the changing needs of children and families.
Management Responsibilities
- Provide leadership in the absence of the Team Leader or Clinical Lead, ensuring effective service delivery.
- Accept referrals and discharge patients from your own caseload.
- Reassess children, ensuring regular formal review of the caseload with the Team Leader, documenting and evidencing all reviews.
- Maintain standards in clinical quality and governance as central to the function and delivery of the service.
- Provide advice and guidance to patients, families, carers and other team members.
- Act as an effective role model for other team members, students and professionals.
- Participate in research and audit relevant to the service with the Team Leader or Clinical Lead.
- Manage service resources (equipment, staffing, supplies) in conjunction with the Team Leader or Service Manager.
- Develop policies, procedures and guidelines with the wider multidisciplinary team.
- Ensure staff maintain organisational standards in record keeping.
- Conduct risk assessments for your clinical caseload, action‑planning effectively in response to risk, and communicate concerns to line manager.
- Manage informal complaints from service users, raising recurrent themes or concerns with the Clinical Lead and Service Manager and assisting with formal investigations as required.
- Demonstrate critical thinking, problem solving and decision‑making skills.
- Maintain positive working relationships with colleagues.
- Accurately record and maintain client records daily using electronic and paper systems.
Training & Development
- Maintain and update knowledge and skills in relation to the post.
- Identify and undertake specific training to meet NMC Scope of Practice and service development needs.
- Participate in teaching of students and other staff within the clinical and university environment.
- Provide mentorship to staff as required, ensuring they possess necessary knowledge and skills.
- Maintain professional development in line with post‑registration ongoing education, including revalidation.
- Act as a link nurse and lead clinically in areas of expertise, attending courses and meetings as delegated by the Team Leader.
- Participate in reflective practice, clinical supervision, critical incident analysis and debriefings.
Management & Leadership Responsibilities
- Develop and empower all team members to perform to high standards and innovate.
- Ensure supportive staff management arrangements, carrying out personal development reviews for direct reports and ensuring annual PDRs with specific objectives and effective personal development plans.
- Promote equality and diversity, ensuring specific equality objectives are included in PDRs.
- Develop a culture that meets the standards of Improving Working Lives and Investors in People, measuring staff well‑being and implementing improvements.
