Greenhill Park Medical Centre is a NHS GP surgery in North West London, serving patients in the Harlesden and Neasden areas. It forms part of a group of GP services alongside Neasden Medical Centre and St Andrews Medical Centre, all operating under the Neasden-Greenhill Park and St Andrews Medical Centres partnership.

Main duties of the job

As a GP practice, the centre provides standard NHS primary care including:

• Appointments with GPs and nurses (face-to-face or telephone).
• Repeat prescriptions and sick (fit) notes.
• Test results and referrals.
• Vaccinations, health checks, and wellbeing support.
• Online eConsult service (to request advice or care via an online form).

Job Purpose

The Nurse Triage will provide clinical assessment and triage of patients presenting to the GP practice with urgent or acute health needs. The role aims to improve access to timely care, support patient flow, and ensure patients are directed to the most appropriate service.

The post holder will assess patients via telephone, video, and face-to-face consultations, providing clinical advice, treatment, onward referral, or discharge as appropriate.

Experience

• Work which would prepare for making clinical decision on patients' care.
• Work in acute settings, i.e. A & E

Qualifications

• Registered Nurse with NMC registration.
• Relevant post-registration experience (typically 23 years).
• Experience in autonomous clinical decision-making and triage.
• Excellent communication and consultation skills.
• Ability to assess risk and prioritise workload effectively.
• Experience in urgent care, emergency care, or primary care settings.
• Competent IT skills and experience using electronic patient records.
• Minor illness or advanced clinical practice qualification.
