• Ability to assess, diagnose, treat, and prescribe within the scope of practice.
• Experience in managing palliative and end-of-life symptoms.
• Good communication skills for liaising with community nursing and other teams.

Responsibilities

• Undertake home visits for allocated patients.
• Assess and treat patients in their own homes.
• Document consultations and handovers accurately.
