A healthcare provider in Chester is looking for a professional to manage rehabilitation assessments and a caseload of patients in their Inpatient Rehabilitation Services. The ideal candidate will ensure high-quality, patient-centered care and may assume Clinical Lead duties as needed. This role emphasizes communication and effective treatment management to meet rehabilitation goals.
An exciting and unique opportunity has arisen to be a part of the Rehab Case Management Team working within their Rehab Access Function.
In this role, you will be responsible for developing and delivering tailored assessments for individuals undergoing rehabilitation, both in community and inpatient settings. You will manage a caseload of patients within our Inpatient Rehabilitation Services, ensuring high-quality, patient-centred care.
In addition to your clinical responsibilities, you may be required to step in as a deputy for the Clinical Lead during periods of absence, such as annual leave or sickness. This will involve serving as the primary point of contact for both internal and external agencies related to the Rehab Access function, ensuring smooth operations and continuity of care.

All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
