Overview

West London NHS Trust is looking for a skilled and enthusiastic Mental Health Practitioner to join the H&F Central MINT (Mental Health Integrated Network Team). The role will be key in developing pathways of care within the local community mental health team and will support individuals to access mental health services wherever and whenever they need it.

Responsibilities
  • Develop and improve local care pathways for the Mental Health Integrated Network Team.
  • Support people in accessing and managing mental health services within their local community.
  • Collaborate with local MINT teams to strengthen relationships and service delivery.
  • Provide continued education and career progression opportunities within the team.
Qualifications
  • Registered Mental Health Nurse, Social Worker or Occupational Therapist with experience in community services.
  • Prior experience working across a range of community mental health settings and with a variety of mental health diagnoses and needs.
  • Knowledge and experience delivering brief interventions to patients with mental health needs.
  • Good written and verbal communication skills and the ability to use these to strengthen local relationships with the local MINT teams.
