Job description

We have an exciting opportunity for Primary Care Nurse to join our Primary Care team at HMP Kirkham, a category D Prison located in Kirkham, Preston. We are looking for someone to support and coordinate a small team by managing planning, overseeing coordination and supporting junior team members. The prison has its own Healthcare department where you will be based and the primary care team consists of a team of 10. HMP Kirkham has a capacity of 700 males.

Hours
  • Full‑time 37.5 hours per week, Monday – Sunday
  • Various shifts between 8:00 am – 7:00 pm
  • Occasional weekend rotations as required
Responsibilities
  • Work within a multi‑disciplinary team carrying out comprehensive assessments, initiating and reviewing care plans, and developing treatment options.
  • Undertake medication rounds and assess and treat patients who may have undergone traumatic experiences and may be distressed.
  • Assess patients’ health needs using an evidence‑based framework and risk‑assess using sound clinical knowledge.
  • Work with individuals providing advice, support and information and manage a caseload of patients.
  • Deliver non‑judgemental care that makes a real difference.
  • Monitor therapeutic responses to medication and potential side effects, liaising with prescribers as required.
  • Inspire excellence by forging strong links with our partners and stakeholders.
  • Promote best practice, clinical supervision, evidence‑based care.
  • Share knowledge, skills and expertise to ensure safe care, build strong multi‑disciplinary teams and support each other to do a great job.
Qualifications
  • Qualified Mental Health Nurse – NMC registration.
  • Evidence of continued professional development.
  • Experience of working with patients face‑to‑face who have complex mental health conditions.
  • Experience of psychosocial interventions, group work and awareness of substance misuse.
  • Ideal: experience in a hospital, community or prison environment.
