A leading healthcare provider in Leeds is seeking a Clinical Lead for Children's Mental Health and Safeguarding. Job description

The PCN have identified a requirement for a Clinical Lead for Children and Young Peoples Mental Health and Safeguarding in the PCN. This is to provide extra support to some of our young patients and their families, and to further enhance partnership working. It is expected in the first 6 months this post will review what needs to be undertaken at PCN level to ensure that we are connecting with all relevant partners and there is a clear understanding of the pathways available for patients. The PCN is also going to establish a Children and Young Peoples Social Prescribing Service. This new role will be responsible for setting up this service in the first 6 months and will be the clinical lead.

The post holder will be responsible for the operational and clinical delivery of providing support/ brief treatment and/or advice to children and young people who appear to be falling between the gaps. At present the GPs are noting a high level of patients coming to the GP that feel they are lost in the system, and there is not the expertise in the PCN to support this growing number of patients. The role will work in an integrated way with the Autism and ADHD neurodevelopmental pathway for children and young people. Working in a better integrated way has the potential to have a positive impact on improving outcomes and reducing inequalities for groups of children and young people and their families at higher risk of poorer mental health outcomes. This could be through risk stratification of the PCN population and using this to target initiatives and resources and also ensuring culturally responsive delivery.

The role will ensure that high quality and safe services are delivered as well as helping patients and families navigate the system and to identify the right pathways by working closely with other colleagues. The post holder will not hold a caseload. They will also be responsible for developing the new Children and Young Peoples Social Prescribing service in the PCN as well as being the clinical Lead for this service and the Safeguarding Lead for the PCN.

NHS England has significant ambitions for Primary Care Networks, with the expectation that they will be a key vehicle for delivering many of the commitments in the long‑term plan and providing a wider range of services to patients.

Woodsley and Holt Park Primary Care Network (PCN) covers a large geographical area in Leeds with a diverse population. There are 10 practices within the network with a population of over 115,000 patients. Primary care networks consist of groups of general practices working together with a range of local providers, including community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations. They employ a wide range of health professionals including pharmacy technicians, pharmacists, first contact physiotherapists, social prescribers, dieticians and podiatrists to provide appropriate care for patients and to allow GPs to concentrate on the more complex patients.
