A health care provider in Cumbria is seeking a Living Well Coach to support individuals with long term health conditions in improving their self-management skills. The role involves coaching, motivational interviewing, and personal goal setting in various environments. Working within Keswick and Solway GP practices, Eden GP practices and working closely with the wider ICC team, the Living Well Coach (LWC) will work within the Wellbeing Service to provide a coaching service for individuals with long term health conditions and psycho-social needs engaging them to set personalised goals and find and access services and resources best suited to meet their needs and circumstances.
Depending on the individual the assessment might include low level clinical screening, wellbeing assessment, review of functional abilities, assessment of social / housing / employment circumstances and gaining understanding of social and support networks.
LWCs will develop skills in active listening, motivational interviewing and appropriate psychological care skills including CBT skills, acceptance & commitment therapy, mindfulness and social prescribing. Training and on-going clinical supervision will be provided by the Physical Health Psychology Department within North Cumbria Integrated Care NHS Foundation Trust (NCIC).
Using their coaching skills to support individuals identified by the GP practice and wider Integrated Care Community (ICC), Living Well Coaches (LWC) work alongside people who want to improve their skills in self management of their own health and wellbeing, the coach will listen to what is important to the person and help them to identify and commit to step by step activity, building towards their improvement goal.
Although the base will be within one Integrated Care Community (ICC) the post-holder will be expected to work in a range of environments across Keswick & Solway ICC and Eden ICC, including visiting people in their own homes, in other GP practices or community health/ care settings. Individual LWCs will be allocated to one or two practice(s) allowing continuity for patients and direct engagement with the GP practice team.
The interview date will be Monday 15th December and interviews will be held at Wigton Hospital, Cross Lane, Wigton Cumbria, CA7 9DD. A values based interview session will be held lasting from 09:30-17:00 (timings are approximate and will be confirmed nearer the time). Candidates will be expected to attend for the duration of the session.
