Qualifications

  • Minimum 2 years post qualification experience.
  • Experience in primary care or medicines optimisation.
  • Mandatory registration with General Pharmaceutical Council (GPhC).

Responsibilities

  • Manage patients with long-term conditions through medication reviews.
  • Provide clinical input for acute and repeat prescriptions.
  • Conduct structured medication reviews for polypharmacy and high-risk patients.
  • Work closely with GPs, nurses, and other healthcare professionals.
