Experience Required (ESSENTIAL): Minimum 2 years within a Domiciliary/Complex Care Coordinator role. Hands‑on care experience is a plus. Please do not apply if you have not worked in a similar role. Care Assistants/Support Workers who do not also have experience as a Care Coordinator will not qualify for this role.

About the Role

We are looking for an experienced and compassionate Care Coordinator to join a respected health and social care provider, delivering domiciliary care, complex care, and outreach support services across Stockport and Greater Manchester.

Key Responsibilities
  • Plan and coordinate care visits, ensuring client needs are met efficiently.
  • Build and maintain strong relationships with service users, their families, and care teams.
  • Lead, support, and supervise a team of care workers, ensuring high‑quality service delivery.
  • Conduct risk assessments, develop care plans, and carry out regular service reviews.
  • Respond quickly to queries, emergencies, and changes in care requirements.
  • Ensure all work complies with CQC standards and company policies.
What We're Looking For
  • Previous experience as a Care Coordinator or in a similar role within health & social care sector is essential.
  • Strong organisational and communication skills.
  • A caring, professional, and person‑centred approach.
  • Knowledge of not just domiciliary but also complex care.
  • Confident in using scheduling software and handling administrative duties.
  • Full UK driving licence and access to a vehicle (required).
  • Willingness to travel to clients to carry out needed checks.
  • Possibly cover care work if absolutely necessary. (This is extremely unlikely but willingness if needed is a must).
