Overview

Our Enablement and Urgent Care services are responsive and flexible with the aim of supporting people to develop and maintain their independence within their home environment. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. Car owner/driver is essential (subject to the Equality Act 2010).

You should note that public transport is rarely available option as your role will involve travelling to rural areas, often with limited public transport.

About the Service and Location

This new Service has been designed to offer a wrap round supported discharge from Hospital or to provide an integrated approach to prevent hospitalisation. Initially, the service will operate within the Thanet and Canterbury areas with a base at Westbrook House in Margate. It will operate 7 days a week, 8am to 8pm.

Someone who would welcome the chance to create an excellent team and support the staff through any challenges. Someone able to monitor performance as the service progresses and make changes to suit the needs of the service.

Additional Notes

The role will be office based in Dover with the flexibility of working from Trinity House in Ashford and Westbrook House in Thanet. You will be employed by Kent Community Health NHS Foundation Trust, but equally accountable to Kent County Council.
