A leading healthcare provider in Bangor is looking for a Home Therapies Specialist Nurse to join the Ysbyty Gwynedd Home Dialysis team. The role includes training and supervising patients in home dialysis, collaborating with multidisciplinary teams, and ensuring quality care. Home Therapies Specialist Nurse BCUHB West
This is an exciting opportunity to join the BCUHB Ysbyty Gwynedd Home Dialysis team who are responsible for haemodialysis and peritoneal dialysis patients in our area.
You will be joining a highly motivated team of nursing working closely with a multidisciplinary team in the renal unit in Bangor & Alltwen.
The role will involve the training and supervision of home haemodialysis and peritoneal dialysis Patients, both in the Renal Unit and within their own homes. This will require the specialist nurse to provide care and supervision for a case load of home dialysis patients, working within the Renal Unit MDT.
Our Patients are closely monitored by our team within their home environment and closely monitored in the joint consultant Nephrologist and Nurse lead clinics. Other requirements of the role will include the ability to assess the fluid status of a Patient, to assess adequacy of dialysis, monitoring dialysis access and their dialysis technique, together with their general well being and the way in which they are coping with dialysis at home. It will also involve the monitoring of equipment and treatment programmes, monitoring of blood results and haemodialysis water quality. Training will be provided to ensure expertise in this area.
