A leading hospice organization in Plymouth is seeking a Healthcare Assistant for its End-of-Life Urgent Care Service. This role involves supporting community-based end-of-life patients, helping them achieve their preferred place of care and preventing unnecessary admissions to secondary care. Ideal candidates will be enthusiastic and motivated individuals keen on making a difference in people's lives during challenging times. Informal discussions are encouraged with the clinical leads.

St Luke's is delivering on its vision of a Hospice without Walls with the support of its End-of-Life Urgent Care Team, as a Healthcare Assistant for the End-of-Life Urgent Care Service you will assist in supporting community-based end of life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.
Whilst this role can be busy and challenging, there is great reward in helping make a real difference at the end of a person\'s life by caring for them in their own homes and preferred place of care/death.
