An exciting opportunity for Health Care Assistant's has arisen to join our Ward 1 - Escalation Team Department. We require staff who have a genuine interest in helping caring for acutely unwell patients as part of a multi-disciplinary team as well as having excellent communication and interpersonal skills. Ward 1 is a 12 bedded low acuity ward, accepting patients requiring minimal medical intervention.

Ward 1 works closely with all members of the team to ensure competent safe delivery of care and achievement of specific role dependent duties within your scope of training and competence. We communicate compassionately and effectively with patients and their families whilst maintaining confidentiality. We deliver personal care to patients whilst maintaining their privacy and dignity and support with patient rehabilitation. We provide safe, clean and friendly care and recruit to our SCOPE values.

Responsibilities
  • Deliver compassionate and effective care to patients within the scope of training and competence.
  • Communicate with patients and their families while maintaining confidentiality.
  • Provide personal care, preserving privacy and dignity, and support patient rehabilitation.
  • Work with the multidisciplinary team to ensure safe and competent delivery of care and the achievement of role-dependent duties.
  • Follow Trust standards, policies and protocols; observe, assess and respond to patients’ needs and escalate concerns as required.
  • Support patients with activities of daily living, including mouth care, personal hygiene and toileting, demonstrating person-centred care.
  • Safely assist with moving and handling and use of patient equipment (e.g., hoists and wheelchairs) as required.
  • Undertake delegated procedures under the supervision of a registered healthcare practitioner within training and competence limits (e.g., patient moving and handling, personal care).
  • Support feeding, monitor dietary and fluid intake, and report changes to nursing staff.
  • Manage patient property per Trust policy; monitor and order supplies as appropriate.
  • Demonstrate duties to new starters and contribute to the learning environment.
  • Work in line with Trust policies and procedures; contribute to a safe, healthy working environment.
  • Follow duty of care principles and act in the best interest of individuals to prevent harm.
  • Collaborate with individuals, carers/families and the wider healthcare team.
  • Maintain and develop own skills and knowledge through development activities; prepare for appraisal and participate in annual reviews.
  • Participate in team handovers.

  • Record nutritional intake and fluid balance; report changes to nursing staff.
  • Ensure patient property is managed per Trust policy and maintain supply levels.
  • Demonstrate duties to new starters and contribute to the learning environment.
  • Maintain personal development and participate in appraisal.
