Job description
Overview

Chartridge Ward is a 22-bedded ward within the Integrate Elderly Care Division. We care for individuals who are medically optimised for discharge but who require nursing support until the appropriate community care can be arranged. Patients may receive a Package of Care in their own home or be placed in a Nursing or Residential Home. Our patients often have multiple health needs and chronic conditions.

Responsibilities
  • Provide high quality nursing care to patients on Chartridge Ward.
  • Demonstrate excellent communication and inter-personal skills and work effectively within a multi-disciplinary team (nurses, physiotherapists, occupational therapists, doctors, pharmacists, social workers & discharge coordinators) as well as independently when required.
  • Participate in training and clinical supervision opportunities as available.
  • Support the discharge planning process in collaboration with the multidisciplinary team.
Qualifications
  • Good standard of English and numeracy skills.
  • Excellent communication skills and a caring attitude towards older people.
  • Demonstrated ability to work in a team and independently.
  • A passion for looking after older people and providing compassionate care.
  • Shortlisting may include a numeracy and literacy competency test; interview if standards are met.

  • PPE is worn in accordance with infection prevention and control procedures.
