Job description
Overview

You will be a core member of the Urgent Care West Kent team based at Unit G, Maidstone where you will deliver both clinical and non‑clinical care to patients/clients. You will also be key in supporting patients with their own self‑care and independence. This service is part of the Urgent Care Services which provides a two‑hour response to patients in crisis at home.

Our aim is to prevent unnecessary hospital admission and facilitate early discharge from acute hospitals. We also provide unscheduled care out of hours for patients who may be at the end of life or experiencing an urgent need for healthcare, treating acutely unwell people that would otherwise require hospital admission and supporting hospital discharges.

The service runs 24 hours a day, 365 days a year, providing nursing care within patients' homes across West Kent. Driving is essential for this post.

Responsibilities
  • Recording your actions accurately in patient notes
  • Basic observations
  • Personal care – washing and dressing
  • Catheter care – changing catheter bags and catheters
  • Mouth care
  • Stoma care
  • Supporting fluid and nutrition
  • Calibration of blood glucose machines and taking blood glucose levels
  • MRSA swabs
  • Risk assessments: falls assessment, nutrition and hydration, skin integrity
  • Sub‑cutaneous injections (e.g. insulin injection)
  • Venepuncture
