A friendly medical practice in Caernarfon is expanding its clinical team by seeking a committed GP. The role involves patient consultations, chronic disease management, and home visits, supported by a fantastic nursing and administrative team. Patient Consultations & Diagnosis: Assessing undifferentiated symptoms, providing diagnoses, and conducting physical examinations in person or by phone.
Chronic Disease Management: Developing and managing long-term care plans for patients with conditions like diabetes, asthma, or heart disease.
Preventive Care & Health Education: Promoting health through screening, immunisations, and lifestyle advice.
Referrals & Coordination: Referring patients to hospital specialists or other healthcare professionals when necessary.
Administrative & Clinical Tasks: Maintaining accurate, confidential patient records, signing, and managing repeat prescriptions, and ensuring legal compliance with health regulations.
Home Visits: Visiting patients at home or in care homes when they are unable to travel to the surgery.
