Job Summary
We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. This role is for a fixed term to support the practice during a period of long term sickness and phased return to work. The position requires approximately 15 hours per week, mainly on Tuesdays and Fridays, with the possibility of alternative days but no guarantee beyond those two.
Responsibilities
- Deliver comprehensive patient care, including conducting health assessments.
- Health‑promotion and well‑being, including the management and treatment of long‑term conditions.
- Assist in the administration of vaccinations, injections, and other medical treatments as prescribed.
- Review medication for effectiveness and support adherence to treatment.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
- Educate patients on health management, disease prevention, and wellness practices.
- Participate in ongoing professional development and training to enhance clinical skills.
- Support QOF targets.
Skills
- Strong knowledge of patient care principles and practices.
- Proficient understanding of human anatomy and physiology.
- Excellent communication skills, both verbal and written, to effectively interact with patients and colleagues.
- Ability to work efficiently in a fast‑paced environment while maintaining attention to detail.
- Compassionate approach towards patient care with strong interpersonal skills.
- Capability to handle sensitive information with confidentiality and professionalism.
Qualifications
- NMC Registration (required).
Experience
- Experience of working in primary care.
- Skilled in management of patients with long‑term conditions such as COPD or diabetes.
- Knowledge of health promotion strategies.
- Knowledge of QOF.

Pay
Depending on experience: £37,500.00‑£42,500.00 per year.
Contract
Fixed term, 12 months.
