Overview

The Jean Bishop Integrated Care Centre (ICC) is the first of its kind in the UK and has operated since May 2018, delivering innovative, anticipatory, patient-tailored care to over 4000 frail residents in Hull and surrounding areas. The service has received national recognition, including a Health Service Journal Award, and continues to receive positive patient feedback. The ICC has been featured on NHS England’s website as a leading case study in the Ageing Well section. The team is committed to transforming care for older people in Hull.

Responsibilities

• Provide support to the MDT team in the delivery of nursing and therapies interventions for treatment of older people and those with frailty.
• Lead and drive the clinical support worker team to support service delivery and address individual team member development needs.
