Enhanced Nurse Practitioner - Andover PCN

Andover Primary Care Network is looking for an EnhancedNurse Practitioner (ENP) who will work autonomously within their professional scopeof practice, providing care for patients across all 5 of our core networkpractices via face to face, telephone consultations and home visits.

You will need to be anenthusiastic, caring and motivated professional with excellent clinical,organizational & communication skills.

Main duties of the job

As part of the PCN multidisciplinary team youwill be responsible for Clinical Delivery of services, Education and Supervision of the Health Hub team, provide Administrative Leadership and Support by being the designated infection preventionand control (IPC) Lead for the PCN.

Job responsibilities

Job Summary:

Aspart of the PCN multidisciplinary team you will be responsible for thefollowing:

ClinicalDelivery

The EnhancedNurse Practitioner will serve as the Clinical Lead for the Andover Health Hub,providing expert clinical support, oversight, and guidance to the GeneralPractice Assistants (GPAs) and Care Coordinators. In this capacity, thepractitioner will facilitate safe and effective clinical decision-making withinthe team, ensuring high standards of patient care.

In addition,the Enhanced Nurse Practitioner will act as the Clinical Lead during COVID-19vaccination campaigns, overseeing the delivery of safe and efficientvaccination services in line with national and local guidance.

A key elementof this role will include supporting the frailty pathway across the PCN. Thepractitioner will contribute to frailty management, including undertakingStructured Medication Reviews, participating in multidisciplinary frailtyreviews, and completing home visits where clinically appropriate.

To support thefive core network practices, the practitioner will also provide one clinicalsession every other week, tailored to practice needs and individualpractitioner competencies. These sessions may be delivered on site withinpractices, from the central Health Hub, or remotely (for example, within carehome settings), ensuring a flexible and responsive model of care.

Education& Supervision

The EnhancedPractitioner will provide clinical supervision, education, and skills development for the Health Hub team, ensuring that GPAs and CareCoordinators have the necessary clinical competencies to fulfil their roleseffectively. The practitioner will also support workforce development byidentifying learning opportunities and fostering a culture of continuousprofessional growth.

AdministrativeLeadership & Support

Thepractitioner will act as the Infection Control Lead for the Health Hub,ensuring compliance with infection prevention and control (IPC) policies and procedures in alignment with national and local IPC standards.

Additionally,the practitioner will work in collaboration with the non-clinical Health Hub Manager to review and update relevant policies, protocols, and standardoperating procedures (SOPs), ensuring they reflect current clinical bestpractices and regulatory requirements.

This post attracts 33 days of paid annual leave and verygenerous NHS Pensions.

In order to work at this level, you should have

a. a degree innursing or from a registered nurse degree apprenticeship (RNDA);

b. registeredwith the Nursing & Midwifery Council

c. hasa post graduate qualification at level 7 or above relevant to their area ofenhanced practice, for example in wound care, diabetes plus respiratory andCVD, dementia, women's health and public health and population healthmanagement, or other area of enhanced practice.;

d. is working at anenhanced level of practice as described in the Primary Care and GeneralPractice Nursing Career and Core Capabilities Framework

e. works across the PCN, in services thatenhance the core general practice offer.

Key Responsibilities:

Theenhanced nurse practitioner will be working across the Andover Primary CareNetwork doing a combination of practice work and at the Health Hub deliveringthe following key responsibilities

a)worksas part of a MDT within the PCN;

b)consultswith patients, their family, and the multi-professional team to undertakeassessments of patient need and devise and evaluate complex care plans.

c)evaluatesand analyses clinical problems using their clinical knowledge, seeking out andapplying relevant evidence, enhanced techniques, interventions, and equipmentto make clinical decisions.

d)deliversenhanced clinical care in the context of continual change, challengingenvironments, different models of care delivery, innovation and rapidlyevolving technologies using analysis and their underpinning knowledge to managecomplex interventions.

e)teachesand advises patients and their families on how to manage their condition orsupport the multi-disciplinary team to do so.

a)participatesin clinical audits and research projects and implements changes as required,including the development, and updating of practice protocols / guidelines andprocedures locally.

b)workswithin national and local protocols where these exist.

c)recognisesboundaries of their practice and know when and to whom patients should be referred.

d)usingthe principles of delegation, delegates work to other members of the MDT andtake responsibility for the delegated activity as appropriate.

e)demonstratesinitiative and are creative in finding solutions to problems.

f)holdsresponsibility for team performance and service delivery; and

g)providessupervision to trainee nurse associate, nurse associates and general practicenurses.

Wider Responsibilities:

1.Supportwith the delivery of structured medication reviews, focusing on polypharmacyand deprescribing.

2.Facilitateand support the development and delivery of innovative training and educationto staff, to improve competence and confidence, in line with best practice andguidance related to area of expertise.

3.Participatein and/or lead formal training events promoting best practice in area ofexpertise.

4.Provideprofessional and clinical leadership and mentorship, acting as a clinical rolemodel on the delivery of evidence-based practice.

5.Adviseand support on the development and delivery of work streams relating to area ofexpertise as part of the PCNs strategy, where appropriate.

6.Workproactively with key local and national stakeholders to develop more integratedcare pathways and holistic models of care, through dissemination of specialistskills and knowledge.

7. Contribute to the delivery of local policy,procedures and protocols for the PCN to improve identification, assessment,ongoing support for patients and their family carers.
