Job description
Overview

An exciting opportunity has arisen for a part-time (15 hrs/week), permanent End of Life Care Facilitator post based in the Palliative Care Service at the Homerton Hospital site. The service provides specialist palliative and end-of-life care to adult patients and people important to them at the hospital and Mary Seacole Nursing Home.

Responsibilities
  • Provide expert nursing support in end-of-life care (EoLC), working alongside and supporting generalist staff.
  • Act as a clinical role model, offering education, advice, and emotional support to patients, families, and staff.
  • Assist teams in recognising dying, managing symptoms, and avoiding inappropriate interventions.
  • Facilitate advance care planning and support patient and family involvement in decision-making.
  • Lead and influence improvements in palliative and EoLC across the Trust.
  • Develop, deliver, and evaluate education and training programmes for non-specialist staff.
  • Mentor students and early-career clinicians in providing quality EoLC.
  • Collect and analyse data to monitor and improve service delivery; contribute to audit, quality improvement, and research.
  • Support implementation of Trust-wide EoLC strategy, fostering collaboration, shared governance, and best practice.
  • Maintain professional standards and continuous professional development in line with NMC requirements.
