Job description
Embedded Prescribing Pharmacist (Medicine)

The closing date is 30 September 2025

We are looking for a dynamic clinical pharmacist, with extensive NHS post-registration experience, to work as an independent prescriber within a medical specialty at Northern General Hospital.

As an Embedded Prescribing Pharmacist (EPP) you will become fully integrated into the ward team, prescribing both in-patient and discharge medication as well as, following training, writing discharge letters, requesting results, and making referrals, within your specialist area. You will be encouraged to develop ways of practice that suit the intricacies of your directorate and will help to shape the future of your role.

This role is unique in that, as a prescriber, you are additional to the standard ward pharmacy skill mix of clinical pharmacist / medicines management technician, enabling prescribing to be your focus. The role has won the Guts UK Pharmacist Recognition Award 2025 and previously won a Good Practice Initiative award at the European Association of Hospital Pharmacists\' Congress.

You will help to promote the expansion of the role into other specialities and development of other related services, such as the Weekend Discharge Pharmacist, supporting the registrar led Medical Discharge Team (limited commitment, as below).

This is a rare and exciting opportunity to become part of a team that takes independent pharmacist prescribing to the forefront of clinical practice.

Main duties of the job

Provide an independent pharmacist prescribing service to patients within Medicine, ensuring that medicines are optimised for safety, clinical effectiveness, and cost efficiency.

Attend ward rounds, board rounds and huddles to lead on medicines related activities.

Write discharge letters and prescribe accompanying medication to optimise flow.

Work collaboratively with other medical, pharmacy, MDT staff and external agencies to facilitate patient discharge.

Assist the Principal pharmacist, EPP and Discharge, in delivering, developing, and evaluating the service.

Contributing to pharmacy departmental or clinical services, including evenings and weekends. (1 day in 5 weekends)

Note: The role may suit part-time workers, particularly those with experience in Gastroenterology and Respiratory. The specialty assigned to may depend on the expertise of the optimum candidate(s).

Please note this advertisement can be appointed as either a full time or part time opportunity.
