Overview

This is a dynamic and rewarding role where you’ll work across division, multi-agency and interdisciplinary teams to support seamless patient journeys. The Urgent and Emergency Division works to prevent avoidable hospital admissions and facilitate timely, safe discharges, in partnership with health, social care and voluntary sector organisations. You’ll help coordinate and identify patients and ensure they are transferred to appropriate discharge and community pathways. As the service undergoes transformation, you’ll contribute to shaping patient flow and complex discharge planning.

Role and Responsibilities
  • Direct patient flow across the system by supporting timely referrals between Community and Acute providers and Local Authorities.
  • Manage all administrative functions, maintain accurate system and service reports, and maximise flow across the Trust.
  • Coordinate across services to pull suitable patients into the Rapid Care Team and manage the current caseload.
  • Work with the Urgent and Emergency Care (UEC) Division, Virtual Wards, Capacity Hub, Integrated Front Door Team and Urgent Community Response Teams to deliver a coordinated and responsive service that improves patient transitions and overall system flow.
  • Process referrals in line with agreed criteria and maintain an up-to-date caseload on the Redeployed Information (RIO) system; monitor and manage capacity within the division to ensure smooth progression through services.
  • Support safe, timely and effective patient transitions while operating across the UEC division.
  • Work hours are from 8am to 8pm, seven days a week, including weekends and bank holidays on a rota basis.
Role profile and Team

The postholder will work closely with the Clinical Support Coordinator to monitor and manage capacity within the division. The role operates across the UEC division and is a key part of ensuring safe patient transitions.
