A local healthcare provider is seeking a Digital and Transformation Lead for Operations to enhance patient access to care. This role involves overseeing service improvements, managing staff across multiple GP practices, and ensuring compliance with NHS standards. The ideal candidate will have strong project management skills and experience in healthcare settings, as well as the ability to analyze performance data effectively. This is a full-time, permanent position with a salary of £45,000 per year.
This role is for 30 - 37.5 hours per week.
As the Digital and Transformation Lead for Operations, you will support improved patient access to care through the effective set‑up, oversight, and continuous improvement of services. You will contribute to the management of staff, demonstrating a sound understanding of HR processes and procedures, while ensuring contractual compliance and financial sustainability. You will provide clear and timely oversight of services to the PCN Board, ensuring members are appropriately informed to enable effective and well‑evidenced decision‑making.
You will work across 8 practices within Greater Middlesbrough, providing Operational Management support to PCN staff and will be responsible to the PM Working Group.

Support the management of PCN staff, ensuring equitable workload distribution across member practices.
Ensure contractual compliance and contribute to the financial sustainability of the PCN.
Build effective relationships across practices and the wider system to support collaborative delivery of care, including enhanced access models.
Lead and support continuous improvement and quality improvement approaches, including clinically led innovation and integrated working at neighbourhood or place level.
Support the PM Working Group and DES Leads in the implementation of new initiatives and programmes.
Lead and deliver projects that improve service delivery and health outcomes for the PCN population.
Apply best‑practice project and change management methodologies to ensure delivery, benefits realisation, and successful adoption of initiatives.
Support organisational change and provide flexibility to support new and emerging programmes.
Extract and analyse contract and performance data to support delivery and reporting.
Maintain robust project documentation and reporting systems, including dashboards, highlight reports, and risk and issue logs.
Champion best practice, learning, and innovation across the PCN to support continuous improvement.
Analyse complex information to produce accurate project reporting and support informed decision‑making.
Prepare and present regular progress reports to the PM Working Group and PCN Board, including risks, issues, dependencies, and recommendations.
Communicate and engage effectively with PCN leadership, member practices, and stakeholders to support strategy development and programme delivery.
