Role Overview

CLCH Hertfordshire Adult Community Services – CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are a dynamic and forward‑thinking team looking for a band 6 nurse to join our established St. Albans & Harpenden Nursing team as a Deputy Team Lead.

A great opportunity has arisen for an experienced and motivated registered nurse to lead one of our neighbourhood teams under the direction and leadership of the Team leaders.

Your role will be varied and interesting, carrying the responsibility for a patient caseload that involves the assessment, planning, implementation and evaluation of evidence‑based care for patients in their home environment. Previous community experience is essential.

The successful applicant must be someone with a strong commitment to working as part of a team, be approachable, a good communicator, have the ability to work autonomously and be receptive to change.

Responsibilities
  • Set and maintain standards of care and deploy, co‑ordinate and supervise the nursing team and learners as required.
  • Provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
  • Be responsible for the team’s assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8 am–8 pm.
  • Be responsible for the team’s maintenance of patient and other records.
  • Participate in the long‑term management of patients within the locality.
  • Have continued responsibility for a defined caseload.
