Job description
Overview

The Critical Outreach Team at Royal Bournemouth and Poole Hospital, part of the University Hospitals Dorset NHS Foundation Trust, is seeking an Outreach Practitioner who is clinically motivated to support patients at risk of deterioration or other immediately life-threatening events, including risk of cardiac arrest.

The successful candidate will work within a team that supports the clinical care of patients across general medical and surgical wards and other areas, including Interventional Radiology, ED, the Cardiac Intervention Unit, and Critical Care. The team expedites admissions to specialist areas and follows up patients discharged from ICU/HDU daily. They monitor and respond to deteriorating patients via the Trust-wide electronic warning system and attend cardiac arrest calls in conjunction with the Resuscitation Team, providing escort duties for Level 2/3 patients around the Trust.

Responsibilities
  • Support the clinical care of patients at risk of deterioration or life-threatening events.
  • Manage medical emergencies and contribute to the development and implementation of resuscitation policies and procedures.
  • Attend cardiac arrest calls in conjunction with the Resuscitation Team and provide escort duties for Level 2/3 patients.
  • Expedite admissions to specialist areas and follow up patients discharged from ICU/HDU on a daily basis.
  • Monitor deteriorating patients via the Trust-wide electronic warning system and respond as required.
  • Develop high level critical thinking and decision-making skills; work toward performing at an advanced level.
Qualifications and Skills

Trust Context and Relocation Notes

UHD is investing in services across the Trust, with many being transformed and developed after the merger and the New Hospital Programme. Some services may move site this year or next, either temporarily or long term. Recruiting Managers can answer specific questions at interview. On 31/12/25, the CCOT will be on the RBH site only, though this could occur earlier or be delayed. If relocation occurs, employees may not be eligible for excess mileage reimbursement; travel from home to the new base will be treated as a normal commute. Other changes will be managed according to relevant Trust or National terms and conditions.
