The Crays Collaborative PCN is anetwork of 6 GP surgeries whichare Broomwood Surgery, Crescent Surgery, Derry Downs Surgery, Gillmans RoadSurgery, Poverest Medical Centre and St Mary Cray Practice.

The PCN are looking for a Care Coordinator to join their team.

Main duties of the job

Care co-ordinatorshelp to co-ordinate and navigate care across the health and care system,helping people make the right connections, with the right teams at the righttime. They can support people to become more active in their own health andcare and are skilled in assessing peoples changing needs. Care co-ordinatorsare effective in bringing together multidisciplinary teams to supportpeoples complex health and care needs.

They can be aneffective intervention in supporting people to stay well particularly thosewith long term conditions, multiple long-term conditions, and people livingwith or at risk of frailty.

BGPA is a federation of all 43 Bromley practices working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 350,000 patients, 100% of the Bromley population.

BGPAs goals are to work strategically with Bromley practices to help secure the best services for patients whilst working together to support member practices in the challenges of a changing NHS.

BGPA aims to improve the morale of general practice in Bromley by sharing expertise, services and supporting its workforce.

BGPA will make a positive impact on medical services in Bromley by working closely with One Bromley Partners including NHS SEL ICB, local NHS trusts, local providers and patient groups, to improve the delivery of healthcare to the local population.

Job responsibilities

Provide administrative supportfor Clinical Directors

Carry out administrative work tosupport patient clinics such as covid vaccination clinics, and nurse clinicsincluding setting up clinics, booking staff, arranging appointments, liaisingwith other staff and patients when necessary

Liaise with ARRS staff withregards to PCN and practice indicators and targets

Monitor Practices performanceagainst contract targets and support work towards achieving those targets andfeed back to Clinical Directors

Carry out administrative workfor meetings including taking minutes and actioning action points asrequired

Carry out searches on theclinical computer system and report activity data to the Clinical Directorsand Network Manager

Utilise population healthintelligence to proactively identify and work with a cohort of patients todeliver personalised care

Provide co-ordination andnavigation for people and their carers across health and care servicesworking closely with social prescribing link workers, health andwellbeing coaches and other primary care professionals

Assist the PCN ClinicalDirectors and Network Manager with regard to the Network ContractDirected Enhanced Service

Support the other careco-ordinators within the PCN and help develop their role

Organise PCN publicity andevents when required

Maintain good communicationwith PCN practices

Participate in training, as appropriate

Work within organisation policies, procedures and guidelines

Any other duties consideredappropriate to the post

Knowledge and Skills
  • Ability to actively listen, empathise with people and provide personalised support in a non-judgemental way
  • Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies and stakeholders
  • Commitment to reducing health inequalities and proactively working to reach people from diverse communities
  • Ability to identify risk and assess / manage risk when working with individuals
Qualifications
  • Good knowledge and use of the English language
  • Excellent grammar, spelling and punctuation
  • Good standard of education
Experience
  • Experience of working directly in a care coordinator role, adult health and social care, learning support or public health / health improvement
  • Experience of working with a framework of confidentiality
  • Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity)
  • Experience or training in personalised care and support planning
  • Experience of data collection and using tools to measure the impact of services
