Are you an experienced Registered Nurse (complex care), looking to take your skills out into the community? This is your opportunity to join a compassionate, community-focused healthcare provider dedicated to improving the quality of life for people receiving complex care in their own homes.

This role is ideal for nurses with a background in

ICU, HDU, or complex care

This is a full time, permanent role working as a Nurse Case Manager (complex care), in the Merseyside, Cheshire, and Lancashire region. The role is to work Monday to Friday, 9am

5pm, with no nights, weekends, or bank holiday working. The salary is £43,000 per annum. You must be a driver with car for this community role.

What Youll Do

As a Community Registered Nurse, youll play a key role in supporting and overseeing high-quality care for individuals with complex health needs, including those with :

  • Spinal or neurological injuries
  • Acquired brain injuries or trauma
  • Epilepsy, learning disabilities, or genetic disorders

Your responsibilities will include :

  • Assessing, reviewing, and maintaining bespoke care packages
  • Providing hands‑on clinical training and supervision to care staff
  • Offering expert guidance and support to clients and their families
  • Collaborating with health and social care partners (ICBs, hospitals, case managers, and other professionals)
  • Participating in a paid on‑call rota on a rotational basis

About You
  • Registered Nurse (Adult or Paediatric) with current NMC registration
  • Experienced in ICU, HDU, or complex care environments
