Overview

An exciting opportunity has arisen for a full time Community staff nurse to join the District Nursing Service in North Tyneside.

We are looking for an enthusiastic staff nurse to join our district nursing team, this is a fantastic opportunity to become a team member in an exciting service committed to providing patient centred care to patients in their own homes, residential care settings and in clinics. Community staff nurses work autonomously and as part of a team, reporting directly to their line manager, the community nursing team lead, assessing, planning, implementing and evaluating care for a diverse group of patients with a wide range of conditions and health needs.

Key Responsibilities

• Development and delivery of programmes of health and social care.
• Delivery of care and support to patients with a terminal illness and their families including provision of emotional and bereavement support.
• Performance of skilled nursing care including aspects of assessment, wound management, palliative care and catheterisation.
• Performance of diagnostic tests such as Ankle Brachial Pressure Index using Doppler and interpretation of the results to decide further courses of action.
• Performance of procedures requiring fine dexterity and skill such as venepuncture, drug administration by injection and removal of sutures.
• Interpretation of results and communication of these to patients e.g. blood glucose estimation. Explaining aspects of care to patients and carers allowing the opportunity to ask questions and raise concerns.
• Maintenance and promotion of high standards of professional practice including sharing of approaches and experience with others.
• Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability.

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. Philosophy and Leadership Expectations

• Community Staff Nurses are integral members of the district nursing team providing clinical care to a range of patients with acute and chronic health problems. The post includes the performance of a range of clinical procedures and assessments within a primary health care and community settings.
• The Community Staff Nurse works in partnership with patients and carers in the community, providing health education and nursing care within the Scope of Professional Practice. The post-holder is an integral member of the community nursing team working with colleagues to delivery clinical care to patients living in their own homes and within residential care homes.
• Although the post holder has access to advice he / she is expected to work largely unsupervised. In addition, the post-holder is expected to supervise the work of others including auxiliary nurses and student nurses on placement.
• To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
