Job Overview

The successful applicant will be joining an integrated community nursing team delivering holistic high quality nursing care to a dedicated client group in their own homes and residential settings. You will be joining a dynamic and proactive nursing team working in partnership with associated health and social care providers.

Team & Services

The West Community Nursing Team provides comprehensive community nursing care to clients requiring long-term conditions management, complex wound care, or palliative care. Services are delivered in patients’ homes (including residential care) and designated treatment rooms. The team utilises a multidisciplinary approach and collaborates with local providers and other healthcare organisations in Hull and the East Riding.

Key Responsibilities
  • Deliver prescribed care as indicated in the patient care plan and promote best practice of the District Nursing Service.
  • Undertake within scope of practice initial patient care assessments and plan care.
