Job description

The successful applicant will be joining an integrated community nursing team delivering holistic high quality nursing care to a dedicated client group in their own homes and residential settings. You will be joining a dynamic and proactive nursing team working in partnership with associated health and social care providers.

Main duties of the job

The West Community Nursing Team provides comprehensive community nursing care to clients requiring long-term conditions management, complex wound care, or palliative care. Services are delivered in patients' homes (including residential care) and designated treatment rooms. The team utilises a multidisciplinary approach and collaborates with local providers and other healthcare organisations in Hull and the East Riding.

To deliver the prescribed care as indicated in the patient care plan and to promote best practice of the District Nursing Service. To undertake within scope of practice initial patient care assessments and plan care.

Qualifications
  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Knowledge
  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
