An exciting opportunity has arisen for a dynamic staff nurse to join the bank workforce within First Community Health and Care working in the Responsive Services Team.

Responsive Services consists of the Urgent Community Response Team, Intermediate Care Team, Virtual Ward and Referral Hub.

Responsibilities

This post holder will work in collaboration with Primary Care and Secondary Care Colleagues, Urgent Community Response, Intermediate Care (Responsive Services), Community Nursing and other Specialist Health Care Teams across First Community, statutory and voluntary agencies to deliver excellent standards of patient care. Assessing and treating patients who are acutely unwell and requiring support to avoid admission to the acute hospital or those who have recently been discharged home. Providing practical care, education and training for patients and carers admitted to responsive services. You will complete assessments in the patient’s home and undertake treatments including IV medications, wound assessment, and management. You will work closely with the multi‑disciplinary team to promote independence of patients to ensure they can remain in their own home.

The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management and assessment in the community.

Qualifications

Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.

Working Hours

The service runs from 8am‑8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis. There are opportunities for both full time and part time hours.
