Overview

Dorset HealthCare are seeking a Community Staff Nurse to join our vibrant and innovative Integrated Care Team in Mid Dorset, in the District and Community Nurse team in Dorchester. The post is for a permanent position.

You would work alongside our experienced Community Nurse Teams helping to provide a high-quality standard of care for patients in their own homes.

We can be flexible with shift patterns as long as the service needs are met between hours of 08:00-20:00 with some attractive weekend enhanced pay.

Responsibilities
  • Provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy and dignity whilst promoting and embodying the Trust’s values. Undertake assessment of patients’ care needs, and develop programmes of care as part of the wider multi-disciplinary care team.
  • Manage acute and chronic nursing needs, develop strong, therapeutic relationships with patients and work closely with community partners to achieve the best possible health outcomes.
  • Identify care needs, monitor patients’ conditions, record observations and help them develop their wellbeing care plans.
  • Support people to maintain their wellbeing and independence, help to avoid unnecessary hospital admissions, and deliver end-of-life care including symptom management and helping to identify and achieve people’s preferences.
Requirements / Qualifications
  • Enhanced Disclosure and Barring Service (DBS) check is required; the Trust will cover the cost. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of a new check.
  • Employment in this post requires an Enhanced DBS; sponsorship for Skilled Worker visa is not available for this role (applications from candidates requiring sponsorship are ineligible).
  • Structured induction programme, flexible working patterns and pastoral support, with excellent learning and development opportunities (in-house and through links with local universities). Optional preceptorship programme for newly qualified nurses to boost skill development.
