Overview

A rare opportunity has arisen for a Community Specialist Practitioner within our friendly Community Integrated Care Team in Upper Lea Valley. You will work with Community Nurses, Therapists, CPNs and Social Workers to ensure all health and social care needs are met, improving outcomes and reducing unnecessary hospital admissions.

Roles & Responsibilities

The Specialist Practitioner will:

  • Work within the Integrated team of Community Nurses, Therapists, CPNs and Social Workers.
  • Ensure all health and social care needs are met to improve health outcomes, functional status and quality of life.
  • Lead to increased patient choice, prevent unnecessary admissions, and reduce the stay of necessary hospital admissions.
  • Conduct clinical assessments, arrive at a differential diagnosis, start, plan and evaluate care.
  • Act as an excellent communicator, working well individually and within a team.
  • Demonstrate leadership across the inter-professional team.
  • Apply strong organisational and time management skills.
Qualifications & Experience
  • Extensive experience working with older people is required.
  • Evidence of continuing professional knowledge and development.
  • A full valid driving licence and access to a car for business purposes (unless disability qualifies under the Equality Act 2010).
Salary & Benefits

In addition to basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £1,845 per annum (pro rata where applicable).
