A regional NHS foundation trust is seeking a Community Practitioner to coordinate care for service users with complex mental health needs. The ideal candidate will have a professional nursing qualification and experience in community services, focusing on individualised care and support. The position offers a salary range of £38,682 to £46,580 pro rata annually.

The post holder will be responsible for the co- ordination of care needs, assessment, planning and implementation of evidenced based care to a defined group of service users and their carers within the service catchment area whilst under the guidance of senior staff. The post holder will provide professional advice and support to other agencies and members of the multi disciplinary team.
This is a countywide based post providing individualised assessment and care to women whose pregnancy or postnatal period is complicated by serious mental illness which reflects the trusts policy to engage service users and their carers in all aspects of their care and treatment.
