Overview

Pennine Care NHS FT is seeking a Band 5 Community Mental Health Practitioner to join the HMR Community Mental Health Team (CMHT) based at Sudden Resource Centre, Rochdale, working with adults aged 18 years and over. The closing date is 03 October 2025.

Job summary

We have an exciting opportunity for a qualified Nurse to join our Community Mental Health Team as a Care Coordinator. You will work with an experienced, multidisciplinary team and be supported to develop your skills within a Community Mental Health Team setting.

Responsibilities
  • Work within the Care Coordination function of Sudden Resource Centre Rochdale; may work across any function of the three HMR CMHTs as required by service requirements.
  • Maintain a defined caseload and participate in the team's duty rota.
  • Full-time post with core hours Monday to Friday 08:45-16:45; some flexibility required for duty service coverage.
  • PARIS training would be an advantage; training will be provided if required.
  • Work as part of a very experienced team and pursue or develop a career in community mental health.
